Shawnee County Community Corrections-Care Coordinator

Comprehensive assessment, referral and monitoring the clinical substance abuse needs of clients involved with Shawnee County Community Corrections, in order to reduce recidivism, enhance risk reduction and public safety and successfully complete supervision while improving health and safety issues for the client.

Daily Responsibilities:

• Provides initial alcohol and drug assessment, screening, diagnosis and placement services
• Applies a multi-disciplinary approach to client care
• Uses age appropriate diagnostic tools
• Meets access to care standards
• Provides accurate/timely documentation for clinical outcomes
• Participates in quality improvement
• Maintains professional and clinical knowledge of the field
• Represents the organization to the broader clinical community
• Respects clients & stakeholders
• Complies with agency and licensing standards
• Contributes to team effort

Qualifications:

• Masters level or Bachelors level preparation with experience in social work, health care related field, and behavioral health or substance abuse systems; consideration given to those who have extensive experience in the field along with LAC Licensure.
• Licensure as a Licensed Clinical Addiction Counselor (preferred) or a Licensed Addiction Counselor through BSRB must be maintained
• Mental health and/or substance abuse experience preferred
• Excellent interpersonal skills with the ability to communicate effectively through verbal, electronic and written communications
• Ability to evaluate statistical data and attention to detail is required
• Adaptable to change and willingness to adopt new organizational and technology skills as needed
• Experience and ability to use multiple computer applications, including but not limited to, electronic medical record, Microsoft Office Suite applications including Word, Excel and Outlook, and internet based database applications as required
• A valid driver’s license, dependable transportation, and proof of auto insurance must be maintained
