Community-Based Care Coordinator

Provide community-based care coordination to individuals with substance use, behavioral health symptoms/diagnosis, and/or those experiencing employment, legal, or housing issues. Services will focus on meeting individual needs from a holistic, integrated strengths-based model to enhance individual’s cognitive, social, emotional, and coping skill sets. Services are designed to reduce risk, gain access to needed resources, increase stability, and improve one’s overall health.

Community care coordinators serve as a central point of contact and work to link the clients to resources in the areas of Recovery, Housing, Employment, Mental Health and Physical Health. Reducing the barriers and facilitating effective communication in these areas, have proven effective in helping the clients regain stability in their lives, as well as prevent or reduce the need for admission to hospitals, detoxes or jails by directing the client toward beneficial resources in their community. The primary goal of the community care coordination project is to reduce the use of high cost resources and helping clients improve their quality of life.

The care coordinator will focus on the whole person and stress comprehensive assessment, service planning, and service coordination to address multiple aspects of a client’s life. Ability to use a Strengths Based Case Management Model, Motivational Interviewing, and Brief Intervention Skills will be integral to this position. Advocate for client needs in order that opportunity for self-sufficiency is increased.

Positions Available in Garden City, Great Bend, Hays and Lawrence.

Qualifications:

Minimum qualifications include:

  • Masters or Bachelor level preparation with experience in social work, health care related field, and behavioral health or substance abuse systems is preferred.

  • Licensure through the Behavioral Sciences Regulatory Board of KS preferred

    • LAC – Licensed Addictions Counselor

    • LMAC – Licensed Masters Addictions Counselor

    • LCAC – Licensed Clinical Addictions Counselor

    • Consideration will be given to candidates that are in process of obtaining BSRB licensure.

  • Computer skills require proficiency with Microsoft Office Suite including Word, Excel & Outlook

  • Ability to accurately document client progress in electronic medical record

  • Excellent interpersonal skills required – both verbal & written

  • Attention to detail required for documentation and reporting

  • Ability to work autonomously & effectively communicate with stakeholders, clients and staff

  • Bi-lingual skills a plus
