Intensive Care Coordination (ICC)

Intensive Care Coordination services are intensive in nature for individuals with substance use and/or mental health issues, who access high cost resources like emergency departments, in-patient psychiatric hospitals, crisis centers and State hospitals. This service focuses on risk reduction and stabilization, while working to connect individuals with appropriate resources that support healthy living within a community setting.  Heartland RADAC ICC services are available in Barton, Douglas, Ellis, Johnson, Leavenworth, Saline, Shawnee and Wyandotte counties. We partner with Substance Abuse Center of Kansas (SACK), for ICC services in Reno and Sedgwick counties. Please contact SACK directly for ICC services in the those counties at (316) 267-3825. 

 

Eligibility for services varies based on the county of residence. Once a referral is received, our team will meet with the client within 48 hours to initiate comprehensive services that focus on the whole person. These services are initiated with “risk reduction” strategies to begin engaging the individual in the recovery process while simultaneously developing plans to enhance health and safety as well as reducing the barriers that may be standing in the way of the client's success.
