Case Management

Intensive Case Management (ICM)

ICM services are intensive in nature for individuals who have a substance use disorder as well as mental health issues, in-patient psychiatric hospitalizations and/or a history of homelessness. This service focuses on risk reduction and stabilization, and works to connect individuals with appropriate resources to maintain healthy living within a community setting.

Once a referral has been made, the Intensive Case Manager will meet with the client within 48 hours to initiate comprehensive services that focus on the whole person.

ICM services are initiated with “risk reduction” strategies to begin engaging the individual in the recovery process while simultaneously developing plans to enhance health and safety as well as reducing the barriers that may be standing in the way of the client's success.
